Trong thực hành lâm sàng kể cả kê đơn thuốc và phiên giải đơn thuốc cũng như bình luận đơn thuốc tại các cơ sở y tế có không ít lần chúng dã gặp các trường hợp bác sy viết tắt hoặc dược sỹ lâm sàng viết tắt các chỉ định uống hay tiêm thuốc cho bệnh nhân như “bid, po, xl, apap, qhs, hay prn”. Điều này thường rơi vào các đơn thuốc, giấy xuất viện, khuyến cáo thầy thuốc và chế độ sinh dưỡng cho bệnh nhân sau khi ra viện,… từ các bệnh viện nước ngoài sau đó trở về Việt Nam tiếp tục điều trị hay các bệnh viện quốc tế có điều trị bệnh nhân người Việt tại Việt Nam. Show Tại Viện Sốt rét-Ký sinh trùng-Côn trùng Quy Nhơn cũng tiếp nhận khá nhiều bệnh nhân nước ngoài đến khám (Mỹ, Anh, Eucuador, Nam Phi, Bồ Đào Nha, Hà Lan, Thái Lan và Trung Quốc,…) và xin xét nghiệm sàng lọc cũng cho chúng tôi thấy các từ viết tắt này (qua đơn thuốc, kết quả xét nghiệm, hướng dẫn dùng thuốc và chế độ ding dưỡng). Tiếp theo kỳ trước, nhân đây chúng tôi xin chia sẻ các đồng nghiệp y dược một số từ viết tắt và thuật ngữ y học và dược học thường dùng. Nếu nói về từ viết tắt y khoa hay thuật ngữ y khoa khó có thể đầy đủ, nhưng cái khó nhất là làm thế nào chúng ta phiên giải đơn thuốc và chỉ định thuốc trong bệnh án là điều quan trọng nhất, ngay cả một số mã (code) cũng cần phải nhận ra nhanh? Các thuật ngữ viết tắt kê đơn dành cho người bào chế thuốc (apothecary prescription abbreviations) cũng có thể tìm thấy trong phiên bản này mà các dược sỹ bác sỹ và nhân viên y tế đôi khi nhầm lẫn. Thực tế, nếu viết không rõ ràng và từ viết tát bị sai sẽ dẫn đến hệ lụy nghiêm trọng như các lỗi y khoa (medical errors). Một số từ viết tắt có thể làm tăng nguy cơ sai sót y khoa do phiên giải sai, vì thế khi gặp các đơn thuốc đó cần thận trọng và trước khi bắt đầu hướng dẫn bệnh nhân dùng thuốc. Các cơ quan y tế và cơ sở chăm sóc sức khỏe như Cơ quan Quản lý thực dược phẩm Mỹ (FDA_Food and Drug Administration), Hội đồng (The Joint Commisssion) và các Viện thực hành an toàn thuốc (Institute for Safe Medication Practices_ISMP) đã cho ra các thông tin ưu tiên về các từ viết tắt dễ nhầm lẫn (medical shorthands). Điều đó cho thấy vai tò phiên giải cẩn thận các từ viết tắt như thế là rất quan trọng và đôi khi rất nguy hiểm nếu ngược lại. Một số từ viết tắt trong y khoa được đánh máy bởi trên phần mềm máy vi tính, các ký hiệu hay biểu tượng kê đơn và thiết kế liều sẵn có thể đưa đến nhầm lẫn trên từng bệnh nhân, ngay cả thời gian và liều thuốc sử dụng trên từng người bệnh sẽ khác nhau dù cùng tên bệnh (bởi thế mới có câu “điều trị người bệnh chứ không phải điều trị bệnh”. Ngoài ra, khi các từ viết tắt này không rõ ràng, chúng ta sẽ mất thời gian để làm rõ nghĩa để mang lại sự an toàn cho từng bệnh nhân, điều này có thể trì hoãn khâu điều trị mà chúng ta đang cần phải nhan. Ngày nay, nhiều đơn thuốc được kê thông qua máy tính, gọi là đơn thuốc điện tử (e- prescribing), hay hồ sơ bệnh án y khoa điện tử (electronic medical records_EMRs) và ra y lệnh cũng điện tử dựa vào máy tính (computerized physician order entry_CPOE), điều này sẽ giúp rất nhiều về thời gian làm việc cũng như tránh và hạn chế sai sót y khoa. Tuy nhiên, cần lưu ý các đơn thuốc điện tử, các bệnh án điện tử cũng như con người lập trình và đưa thông tin vào nên cũng sẽ có lỗi nếu không thận trọng thì cuối cùng cũng sẽ dẫn đến lỗi y khoa. Từ viết tắt trong tên thuốcCác tên thuốc cũng có thể thường thấy viết tắt. Chẳng hạn, điều trị các ca bệnh phức tạp như trong liệu trình điều trị ung thư có thể viết tắt tên thuốc ung thư. Như báo cáo bởi FDA, một đơn thuốc có từ viết tắt “MTX” thì phiên giải là dùng cả methotrexate (dùng trong viêm khớp dạng thấp) hay mitoxantrone (thuốc điều trị ung thư) và “ATX” được hiểu là shorthand cho zidovudine (loại thuốc điều trị HIV) hay azathioprine (thuốc ức chế miễn dịch). Các loại lỗi này có thể liên quan đến nguy hại nghiêm trọng cho bệnh nhân. Con số nhầm lẫn (Confusing numbers)Các con số có thể dẫn đến nhầm lẫn và sai sót liều thuốc kê cho bệnh nhân dùng. Một ví dụ cho thấy, một đơn thuốc kê “furosemide 40 mg Q.D.” (40 mg mỗi ngày) bị hiểu nhầm “QID” (40 mg 4 lần mỗi ngày), dẫn đến lỗi y khoa chết người. Một ví dụ khác liên quan đến liều thuốc tính bằng micrograms nên luôn luôn có đánh vần đầy đủ chữ vì nếu viết tắt “µg” (micrograms) có thể dễ dàng đọc nhầm “mg” (milligrams), khi đó dẫn đến tăng liều thuốc gấp 1000 lần. Những con số cũng có thể bị phiên giải nhầm đến từng decimal points. Như theo lưu ý của Joint Commission's không sử dụng danh sách (Do Not Use List), một vạch zero (ví dụ 5.0 mg) có thể phiên giải nhầm thành “50” mg dẫn đến liều tăng gấp 10 lần. Thay vì người kê đơn nên viết “5 mg” không có vạch zero hay decimal point sau con số. Tưng tự, thiếu con zero đầu (ví dụ .9 mg) có thể đọc nhầm “9” mg; thay vì kê đơn nên viết ra 0.9 mg” để làm rõ ràng ý nghĩa. Công nghệ ly giải phân hủy thuốc có cải tiến (Modified-Release Technology)Các từ viết tắt thông thường thường được sử dụng theo phương thức cong nghệ cải tiến (modified-release types of technology) cho kê đơn thuốc, mặc dù không có tiêu chuẩn thật toonf tại trong thuật ngữ này. Nhiều thuốc tồn tại trong công thức đặc biệt dạng viên nang hay viên nén. Chẳng hạn, thuốc ER, XR và SR để hấp thu chậm hay thay đổi nơi phân bổ và hấp thu xảy ra trong đường tiêu hóa. Công nghệ ly giải theo thời gian (timed-release technology) cho phép thuốc phân hủy theo thời gian, cho phép nồng độ thuốc trong máu từ từ và có thể số lần một thuốc phải uốngmỗi ngày so với loại thuốc thiết kế ly giải vừa (immediate-release_IR). Các thuốc có công thức bao tan trong ruột (Enteric-coated formulations) như viên aspirine để bảo vệ dạ dày cho phép thành phần hoạt tính qua cầu nối dạ dày thay vì phân bổ trong đường tiêu hóa. Các thầy thuốc lâm sàng, kể cả thầy thuốc, y tá điều dưỡng, dược sỹ và y sỹ nên làm quen với việc sử dụng các từ viết tắt trong thực hành y khoa và thực hành dược phần kê đơn. Tất cả tên thuốc, đơn vị liều dùng và hướng dẫn sử dụng nên viết rõ ràng để tránh nhầm lẫn đáng tiếc. Các dược sỹ nên thành lập nhóm giúp đỡ nhau trong hệ thống EMRs và kê đơn dùng các công cụ điện tử (e-prescribing). Theo hội đồng Joint Commission, các tổ chức chăm sóc y tế có thể phát triển các tiêu chuẩn nội bộ quy định viết tắt y khoa, suer dụng các nguồn tài liệu tham khảo đã ấn bản có các thuật ngữ chuyên dùng và nên đảm bảo rằng phải tránh nhiều từ viết tắt cùng chữ và khác nghĩa. Tuy nhiên, sự quy định nội bộ và tham khảo chuyên môn là chìa khóa quan trọng.
